GULP Business Survey Tracking Directions
The following data are intended to help GULP Local Business Group members track which businesses have completed a survey, have been visited or are reserved by someone for a visit. Please add information about businesses you have have visited or plan to visit in alphabetical order to the appropriate list below. Please use the following format:
Business Name / Your Name(s) / Status Code / Note [if needed] / City [only if NOT Ukiah]
E.g., The Tack Room / Scott C / N
Visit Status Codes
Status Codes are as follows:
C = Complete. Visited and obtained survey
N = Needs follow-up. Visited and left survey, but believe that we should follow-up (e.g., owner was not in or was too busy to discuss the survey at the time).
P = Pending. Visited and left survey. We should wait to see if it is returned.
D = Disqualified. Visited and determined business would not qualify for a listing.
R = Reserved. I am reserving this business and plan to visit it soon.
H = Help Requested. I would like to visit this business but would like a partner, please contact me to coordinate a time.
S = Spanish Language Required. Needs to be visited by someone with Spanish language skills.
